Health Screening Events Paperwork

We typically offer biometric screenings packages which include registration, height/weight/BMI, waist circumference, blood pressure, and a fingerstick (sometimes a blood draw) test to measure cholesterol and glucose. Then we do a 3-5 minute results review session with the participant.
