Injection Site

It is shocking how many nurses (even some of the best!) inject flu shots into the WRONG location. More and more participants are presenting with serious Shoulder Injury Related to Vaccine Administration (SIRVA). In most cases, it is probably the nurse’s fault. PLEASE BE CAREFUL! Also, please review administration info and do not assume your technique is correct. Too many nurses are wrong about this. DO NOT make this mistake!  Review info on over-penetration and SIRVA in The Manual or Flu Shot FAQ and Important Points. Both are accessible here. The FAQ is also included in all shipments.

Event Paperwork

What To Do….  Return it to TotalWellness? Or Throw it Out?

DO Return to TotalWellness:

-On-site Time Log and Acknowledgement of Conduct Agreement (ALL, including agency staff, must sign)
-Event Report (Coordinator or Standard)
-Event Time Extension Form (when applicable)
-Incident Report (when applicable; don’t forget to include “patient’s” name, etc.)
-COMPLETED Consent Forms
-CRL (lab) Sign In Sheet with bar codes (when applicable, e.g. when doing venipuncture)
-CRL (lab) Wellness Sample Tracking Form (when applicable)

Leave for Recycling (DO NOT Return to TotalWellness):

-Unused Consent Forms
-Instructions (general, Shipping, Privacy Screen, Follow-up Counseling)
-Conduct Agreement
-Notice of Privacy Practices
-Staff Evaluation Form (this is ONLY to help you complete the evaluations ONLINE AFTER events)
-Unused Event Time Extension Forms (when event ends at scheduled end time)
-Unused Incident Reports
-Immunization Information Forms (give completed forms to client on-site contacts after all vaccination events)
-Vaccine Information Statements
-Pregnant Women Need a Flu Shot

-Standing Orders (for administering vaccinations and medical management of adverse reactions)

How to Cancel

If you are schedule to work an event and need to cancel, the proper way is to inform us immediately. You must SPEAK with a TotalWellness representative (preferably the Scheduler during business hours).

AFTER HOURSCall 888-434-4358 x 0 to speak with the Night Operator. If the Night Operator does not answer, call 888-434-4358 x 2 to reach the Answering Service and await a callback.​

NEVER email anyone to cancel working an event.

NEVER contact one of our clients/company contacts to cancel working an event.

DO NOT leave a voicemail to cancel working an event.

Call until you SPEAK with someone about being removed from working an event.

TOP 4 TIPS on Receiving Flu Shot Supplies

1. NEVER refuse a shipment from TotalWellness! It’s an expensive mistake. You will ruin any included vaccine! Accept all packages and call TotalWellness for return instructions.

2. If you do not receive your box(es) by the Friday before your event, CALL and SPEAK with someone at TotalWellness as early as possible on Friday. DO NOT email or leave voicemail about it.

3. OPEN box(es) immediately upon receipt and CHECK CONTENTS to ensure nothing is missing or damaged. This will allow time to replace items and prevent an event failure.

4. We do have records of when boxes are delivered to you and will know IF YOU MAKE the MISTAKE of failing to do this.

Flu Shot Consent Forms

DO NOT FORGET to COMPLETE the NURSE’S BOX on Flu Consent/Release Forms

TotalWellness will return forms to you to complete them!

NOTE: TotalWellness must receive all applicable paperwork, including complete Consent/Release Forms, from your past events in order to process your payment. You should simultaneously complete, before or after administration, the “Nurse Box” in the top right corner of the form with the Event #, date, your FULL name, the injection site, vaccine packaging and lot number.

Returning Flu Vaccine

Please return Flu Vaccine immediately after events. We need it right away in order to use it for others and keep it from going to waste.

If your event is on:
Monday: return/ship overnight to TotalWellness on Mon or Tues
Tuesday: ship on Tues or Wed
Wednesday: ship on Wed or Thurs
Thursday: ship on Thursday (for Friday delivery)
Friday: ship on Monday

Onsite Time Logs

  • Be sure everyone, including agency staff, at an event fills in their time and signs the Onsite Time Log.
