Updating Events

  • Any updates and changes to the event need to be made no later than 14 days prior to the event (vaccinations) and 30 days (health screenings and health fairs) not including the event day.
  • This form must be completed and faxed  back to the number on the “Event Update/Change Form”. You may also scan document and email to your account manager.
  • Changes requested less than 14 days for vaccinations and 30 days for health screenings/health fairs prior to the event will incur a service fee per change. Your final invoice will reflect any changes made.
  • Not applicable to Register My Time event changes or updates. Please inquire for any charges related to changes and updates for registermytime.com (online appointment scheduling.)

Please do not call to confirm our receipt of the change and update form. A confirmation email is automatically generated once the event update/change form has been received. The scheduling department determines whether each update/change request can be accommodated.
