You are now ready for the next steps in our application process. TotalWellness will verify your certification(s)/license(s). You will need to upload and attach a copy of the following for verification:

  1. Copy of government-issued photo ID (e.g., Driver’s License or Passport) – Make sure the PDF, JPEG file or copy of your photo ID is LEGIBLE. You may need to lighten and enlarge the copy before scanning.
  2. Copy of CPR and any other certifications.
  3. Copy of nurse or other professional licenses.

* The documents you upload must be a PDF or JPEG file and less than 4 MB in size.

*You don’t have to have a scanner to scan documents anymore. Genius Scan is a great scanner for mobile devices. It allows you to quickly turn photos into JPEG or PDF files. It’s free for iPhone and just a dollar or two for Android and Windows Phones.

Complete your application and attach your documents to your application:

a. Click here to login to system.

b. Log in as a Previous Applicant

c. Click on the “Update My Information” link towards the top

d. Update incorrect information and fill in any blanks (don’t miss Certificates/Licenses and paperwork attachments at the bottom)

  1. Please distinguish your LEGAL First Name from what you PREFER to be called and list your MIDDLE Name if you have one.
  2. If your preferred mailing address is a PO Box or a different address than your home/shipping address, complete the four applicable fields.
  3. Please give us the name and phone number of a person we should contact about any emergencies related to you (e.g., if something happens to you while working with TotalWellness, etc.).
  4. Please enter your Date of Birth and Social Security Number.
  5. Answer the question about whether or not you speak fluent Spanish.
  6. Choose your profession(s). Select multiple professions by pressing “command” (Mac users) or “Ctrl” (PC users) while highlighting each applicable item.
  7. Click on “Add Another Certificate or License” to enter information for any licenses or certificates you have.
  8. To attach a copy of your government-issued photo ID, CPR card and professional licenses, click on “Add Resume & Attachments”. Click “Choose File” and choose the file you would like to upload. Add description and make sure to hit “Attach” at bottom of the page.
  9. Click on “Submit” to submit the changes to your application.
  10. Email RNS@totalwellnesshealth.com to let the recruitment team know that your documents have been attached to your application.

*If for some reason you are not able to upload your documents, you may email them to RNS@totalwellnesshealth.com.

Once verification is complete and approved, you will be emailed directions for the next step, a short online interview. Interview may be done at your convenience. If approved, you will be asked to e-sign the Independent Contractor Agreement and W9. Upon completion, you will then have access to view and nominate for upcoming events in your area.
