Stanislaus Food Products and Corto Olive are pleased to offer a wellness program to increase awareness of the importance of a healthy lifestyle, provide opportunities to practice healthy living, and offer a great incentive for participation to all benefit-eligible employees.

Wellness Program Qualifying Criteria

The program year begins January 1, 2025, and ends October 24, 2025. All criteria must be completed and submitted to TotalWellness by October 24, 2025. To view the qualifying criteria, click here or visit https://stanislaus.lockbenefits.com/.

Biometric Screening Options

1.) On-site Biometric Screening   

This year, our on-site biometric screening has been scheduled for your convenience on October 22. There will be time slots available from 6:00 am – 2:30 pm. The biometric screenings will include measurements of height, weight, waist circumference, blood pressure and a non-fasting screening for total cholesterol, HDL and random blood glucose. This screening will be accomplished with a finger stick, results will be immediate. 

Preparing for Your Screening Appointment

Fasting for at least nine hours prior to your screening is strongly encouraged. To “fast” means that you don’t consume anything except water or medications for nine hours prior to your screening. Fasting helps to ensure an accurate reading of your cholesterol and glucose. Non-fasting testing may cause your results to fall outside of the healthy value ranges required for earning additional wellness points. Drinking water is highly encouraged. If you believe you have a medical condition that makes fasting or undergoing this screening inadvisable, please consult with your doctor.

2.) Your Primary Care Provider   

If you already had a screening in 2025, or would prefer to have a screening with your doctor, you can complete a Primary Care Provider Form. Keep in mind that if your doctor requires an office visit before ordering the screening, you will be responsible for the applicable office visit cost. You should confirm costs with your doctor’s office and insurance company prior to having your screening.

Have your physician complete and give you the Primary Care Provider form. The patient is responsible for submitting the form, not the physician’s office. You must submit the form to TotalWellness by October 24, 2025. After you submit your completed form, you will receive an email confirmation from TotalWellness.

Important Note: Only TotalWellness Primary Care Provider Forms will be accepted. We cannot accept lab results or other forms from the doctor’s office. Please double-check to ensure the form is complete before sending.

There are three ways for you to submit your completed form to TotalWellness:

    1. Online – upload your completed form securely via the TotalWellness website

    1. Fax – send to 402-939-0458.

    1. Mail – send your form to TotalWellness, Attn: Data Department | 9320 H Court | Omaha, NE 68127

3.) LabCorp Screening Voucher  

To have your screening done at a LabCorp facility, complete these steps by October 24, 2025.

  1. Download a LabCorp screening voucher.
  2. Schedule your screening appointment on https://www.labcorp.com. Be sure to make an appointment at a lab that performs Employee Wellness with Body Measurement screenings.  Full instructions on how to schedule your appointment can be found on the LabCorp screening voucher.
    • You do not have to schedule an appointment, but it is highly recommended so that you are sure to visit a facility that performs wellness screenings and so that you do not have to spend extra time at the lab. Online appointment scheduling is available for all LabCorp sites. Same day appointments are available and can be made up to 2 hours prior to arrival.
  3. On the day of your screening appointment bring your appointment confirmation number, screening voucher and a photo ID.

LabCorp will transfer your screening results directly to TotalWellness.

**Your LabCorp results will NOT be automatically mailed to you. You may request that a copy of your results be mailed to you during your appointment at the Lab by checking the boxes in the upper right hand corner or click HERE for instructions on how to view your results online.**

Preventative Screenings

Regular screenings are an important tool to help detect or prevent illnesses, disease or other health problems. You can receive wellness credit for age and gender appropriate preventative screenings you receive throughout the year. See the complete list below.

  • Mammogram – Women over age 40
    • Completed every 2 years
  • Pap smear – Women over age 21
    • Completed every 3 years for ages 21 – 65 or 5 years for ages 30 – 65 with an HPV test
  • Colorectal Screening – Men and Women over age 50
    • Completed every 5 years or 10 years with a FIT every year

There are two ways for you to submit proof of receiving a preventative screening:

  • Online – upload your completed form securely via the TotalWellness website
  • Fax – send to 402-939-0458.

Flu Shot Flexibility

This year, you can get your flu shot (optional) at your local clinic or pharmacy:

Meritain members – Visit your local CVS Pharmacy
Kaiser members – Visit your local Kaiser Clinic

We hope this added flexibility makes it easier to complete your wellness qualifiers on your schedule. If you receive a flu shot, please submit proof via the TotalWellness website to receive credit.

Routine Eye Exam or Dental Checkup

If you had a routine eye exam or dental checkup in 2025, you can now submit proof of a visit to earn one healthy measure toward the Silver or Gold tier level! Proof of either a routine eye exam or dental check can be submitted here,  TotalWellness website.

Noom Coins

Earning 100 new Noom coins helps you earn one healthy measure toward the Silver or Gold tier level! Documentation (screen shot/ picture) of Noom coins earned will need to be submitted toTotalWellness website to receive credit.
