Vaccine & Supplies

Primary contractors are responsible for receiving, maintaining, and returning vaccine, supplies, and paperwork. Vaccine and supplies will arrive in two separate shipments.

Receiving Shipments 

  • Check quantity of items received against packing list
  • Refrigerate vaccine between 36°F – 46°F
  • Freeze cold packs
  • Monitor vaccine temperatures

Vaccine Transportation

  • Utilize provided cooler and materials
  • Pack in the following order:
    • Conditioned cold packs, barrier, vaccine & temperature indicators, barrier, conditioned cold packs

Return Shipment Materials 

  • Return paperwork
    • Completed consent forms, time log, signed standing orders, worksheet
    • If applicable also return incident report, time extension form, roster, station participation log, reconciliation log
  • Return leftover supplies
  • Return leftover vaccine

Return Shipment Method

  • Vaccine must be returned using FedEx overnight
  • Vaccine can only be returned on Mondays, Tuesdays, Wednesdays, and Thursdays
  • A return shipping chart is included in each shipment with additional return shipment information
