Immunization Records System
An email is required. Each person who registers needs to have a unique email because the IR system stores an individual’s vaccine records. You can not share an account with your spouse or parent. You also need a unique code to receive your vaccine. If you do not have an email address, you can sign up for a free email address through Gmail, Yahoo or Outlook.
If you entered the wrong information when completing your profile or want to review what you entered, you can change/view it by logging into the Immunization Records System and clicking on Profile in the right corner of screen. Here you can adjust your demographic information, insurance details and consent form (if you have not yet received the vaccine). You can not change your email address.
You can login to the IR system and view on home page. It’s near the bottom on the left side of the screen. There is also print option. This code is what you will need at your COVID-19 vaccine appointment.
Yes, your code will not change. You will use the same code to check-in at both your first and second dose appointments. You do not need to register again or create a new profile in the Immunization Records system.
The company code is Douglas.
Yes, in order for your COVID-19 vaccine appointment and your profile in the Immunization Records system to be connected, you must use the same email address.
In order for your COVID-19 vaccine appointment and your profile in the Immunization Records system to be connected, you must use the same email address. If you used two different emails, the appointment will not show up. It does not mean you don’t have appointment.
Visit the main login screen at http://ir.totalwellnesshealth.com/profile/signin and click on the “forgot your password?” link. You’ll be directed to enter your email address. A reset password link will be sent to your email allowing you to reset your password.
You can access all your past vaccinations by clicking on the top “Records” tab.
No, you do not need to bring a printed copy of your consent form. When you arrive, the staff will access your online consent using your ID code. Just make sure you have either the text message or email that contains your ID code.

It is very important that your second dose must be from the same manufacturer as the first dose. You can confirm your first dose by visiting the Immunization Records website and clicking on Records. You can also look on your vaccination card.
The Moderna vaccine requires a second shot 28 days after the first. The Pfizer vaccine is 21 days. When you arrive for your first appointment, the staff will make the second dose appointment for you at that time. This helps to ensure the proper time length between shots. If you missed your second dose appointment or need to reschedule, please contact the Douglas County Health Department at at (402) 444-3400.
When the Douglas County Health Department first starting offering vaccines, they were not using our Immunization Records (IR) system. They slowly started rolling out the online system in March. Unfortunately, that means some people’s first dose are not recorded in the IR system. We can’t go back and add previous vaccines.
All COVID-19 vaccines are added to the Nebraska State Immunization Information System (NESIIS). So you can access your Nebraska vaccination records through NESIIS. If you are having trouble accessing your vaccination records through NESIIS, please consult your healthcare provider for assistance.
If you have only received one dose, you can request a new card at your second appointment. If you have already received both doses, the Douglas County Health Department is not replacing cards.

None of the COVID-19 vaccines use the live virus that causes COVID-19. All of these vaccines aim to teach our immune systems how to recognize and fight the virus that causes COVID-19. Sometimes this process can cause symptoms, such as fever. These symptoms are normal vaccine responses that show the body is building immunity.
Yes. To protect yourself and others, follow these recommendations from the CDC:
- Wear a mask over your nose and mouth
- Stay at least 6 feet away from others
- Avoid crowds
- Avoid poorly ventilated spaces
- Wash your hands often
It’s important for everyone to continue using all the tools available to help stop this pandemic. We also don’t yet know whether getting a COVID-19 vaccine will prevent you from spreading the virus that causes COVID-19 to other people, even if you don’t get sick yourself.
You should wait to get your shot. Stay home and watch for symptoms. You should make your appointment for you COVID-19 shot at least 14 days after exposure.
