- Biometric Screening Form Submission
Upload your completed Biometric Screening Form by April 1, 2024. Submitting by March 1 or earlier is highly recommended to allow time for processing.
Before you submit your form, please complete the following steps:
- Print clearly and review your form to ensure all fields are complete.
- Confirm the date of screening is within the following date range: April 1, 2023 – April 1, 2024.
- Provide a valid email address for the individual listed on the form as we will communicate with you regarding the receipt and approval status of your form.
- Ensure that you and your physician have signed your form. These signatures are required.
- Upload each form (employee and covered spouse) separately. Do not upload two forms in one transaction as they will not be processed.
It is the member’s responsibility to ensure the form has been received and approved.
- Watch for an email confirmation of receipt from firstname.lastname@example.org. You will receive an email within 48 hrs. If you do not receive a confirmation email, resubmit your form.
- The confirmation email will inform you if the form was approved or denied. If denied, you will need to complete the missing information and resubmit.
- The form will be processed and data imported into your LiveWell account within 10 business days. Once processed, check your portal to ensure the screening form was received and approved.
