Option 1:   Complete a Physician Form

Please only upload one file at a time. If you combine multiple forms into one file the first form will be the only one processed.

Option 2: Visit a LabCorp Facility

Download your LabCorp Voucher.

Schedule your screening appointment on https://www.labcorp.com. Make an appointment at a lab that performs Employee Wellness with Body Measurement screenings. Measurements taken at your appointment will include height, weight, waist, blood pressure, total cholesterol, HDL, LDL, triglycerides, and glucose.

Option 3: Order a Home Test Kit 

Due to state regulations Home Test Kits cannot be mailed to New York State.

Home test kit includes instructions for drawing a tiny blood sample through a simple finger-prick.

In order to have your results processed by the program deadline you must order a kit by October 18, 2024.

Your home kit will be mailed directly to you within 7-10 business days.
