Screening Option 1: Visit your Primary Care Provider (PCP).

• Completing your screening during your annual physical with your PCP is the most efficient and beneficial way to earn your Wellbeing Reward.

  Note: Baird's medical plan covers 100% in-network preventive appointments with your PCP. Your preventive screening appointment can be scheduled every calendar year and does not need to be 12 months from your last screening visit.

  • Print and bring your form to complete your labs with your doctor. Fasting is recommended.
  • Submit your completed form by secure site or by fax with values signed by your doctor. Expect a confirmation email within 48 hours.
  • Biometric screening results will be added to your B\Well account within 10 business daysAllow 10 business days for results to appear in your B\Well account.

Screening Option 2: Attend an On-site Screening (at participating locations)

Screening Option 3: Visit a LabCorp Facility

• Print your LabCorp Voucher.
• Schedule your screening appointment at a location that offers ‘Employee Wellness with Body Measurement’ screenings using these instructions and searching your zip code.
• If your location does not show search results, clear your cookies and cache by going to your internet settings > security & privacy > clear browsing data > clear cookies and other site data.
• Bring your LabCorp Voucher to your appointment.
• Fasting is required. No food or drink other than water.
• Allow 10 business days for your results to appear in your B\Well account.
