Biometric screenings via Home Test Kits are currently available for Allstate employees. The kit will be delivered to your home where you can collect a small blood sample, complete the health screening form, and send the results back in the pre-paid envelope. Kits must be ordered by 9/1/2026 and returned by 9/30/2026

Follow these instructions:

• Submit order for at-home test kit using the form below. Only one kit per person is allowed.
• Each kit request submitted needs to be completed for the intended recipient of the kit.
• Once your request has been received via this site, you will receive your at-home test kit within 7-10 days.
• Follow the instructions within the kit.
• Mail back your sample.
• You will receive a text message when your results are available to view online.

If you don’t receive a home test kit within 10 days days, please email datasupport@totalwellnesshealth.com. Please double check that your mailing address is correct before submitting the order form. All fields are required.
