Thank you for your interest in the 2026 LIIVeWell Screening Program. You can choose 1 of the following paths to complete your health screening.  See Options 1, 2 or 3 below.

Option 1:   Attending an Onsite Screening

Option 2: Complete a Physician Form – Health Screening Results from your doctor will be from 1/1/2026 to 9/30/2026.

Option 3: Visit a LabCorp Facility – Health screenings can be completed at Labcorp between 1/1/26 to 9/30/2026.

Schedule your screening appointment on https://www.labcorp.com. Make an appointment at a lab that performs Employee Wellness with Body Measurement screenings. Measurements taken at your appointment will include height, weight, waist, blood pressure, total cholesterol, HDL, LDL, triglycerides, and glucose.
