The TotalWellness Personal Health Assessment (PHA) measures the health of your employee group, an important piece to any corporate wellness initiative. Your leadership will receive aggregate level reports that will be essential to developing your wellness strategy. 

The PHA helps participants identify health areas that need improvement. Participants immediately receive a tailored personal health summary including an overall health score and metabolic health syndrome analysis. Companies receive a detailed organizational summary with excellent reporting.

Of course, not all health assessments are created equally. A world-class team, which received two Surgeon General Koop awards, was the mastermind of the TotalWellness health assessment. It measures productivity, absenteeism, and presenteeismrelating to health issues and is extremely user-friendly; employees can complete it online within only 15 minutes. 

The PHA utilizes quantitative biometric drop-down lists, so if a participant doesn’t know his/her exact biometric value the participant can pick from a range: low, medium and high. It incorporates conditional skip logic, meaning participants will only see the questions that they need to answer.  Since it is created by our true wellness company, not an insurance provider, employees can participate with confidence.

A comprehensive personal report summary is generated in PDF format immediately after taking the assessment. Each individual can either save the PDF at that moment, or log back into the assessment later to retrieve their results at any time. 

The individual report includes:

  • Overall health risk score
  • Overall risk scores for cancer, cardiovascular, and mental health
  • Breakdown of overall health risks into modifiable and non-modifiable categories
  • Biometrics summary
  • Readiness-to-change information for each health area
  • Metabolic Health Syndrome risks and description (if applicable)
     

The organizational report includes:

  • Risk factor prevalence
  • Modifiable  and non-modifiable risk factors
  • Individual risk factor prevalence
  • Overall health risks
  • Health risks for cancer, cardiovascular, and mental health
  • Modifiable risk factors to target
  • Metabolic Health Syndrome risks
  • Aggregate on readiness to change for each health risk
