The FIRST STEP to becoming one of our independent contractor medical professionals is to complete the following TotalWellness Online Application.

PLEASE NOTE that you cannot save it and go back. You must complete the Application in one sitting. Allow yourself plenty of time and be prepared with the information listed below. 

VERY IMPORTANT:

*If you do not list four references (two personal and two employer) as requested, your application will not be considered and will be discarded immediately.

*Please do not contact us regarding the stage of your application. Our hiring efforts are focused on areas of highest need. If we choose to continue the hiring process with you, we will contact you with directions on next steps.

TO COMPLETE THE ONLINE APPLICATION,
BE PREPARED WITH THE FOLLOWING INFORMATION:

• Your Legal Name (as listed on Social Security Card, Driver’s License, etc.)
• Nick Name (other primary name you go by, if applicable)
• Professional Title (as it would appear on an ID Badge, e.g., “RN, BSN”)
• Shipping Address (PO boxes are not acceptable for shipping purposes)
• Mailing Address
• Home Phone Number
• Other Phone Number
• Email Address (email is our primary means of communication, you should have your own unique email address; PLEASE NOTE that communications from us may go into your SPAM or other filtered folders, we find this is particularly a problem for Yahoo! email users)
• License and/or Certification Information (types, license numbers,
  expirations, states of licensure, as applicable)
• If applicable, explain any circumstances for which your license, or any other professional license, has ever been suspended, revoked, placed on probation or been the subject of any disciplinary proceeding, or if you have ever voluntarily surrendered your license
• Metro Areas (Primary/largest city closest to your residence and, if applicable, Secondary/other
  nearby area in which you are licensed to work and would like to receive information on PRN work opportunities there)
• Two Previous Employers (employer names, addresses, dates of employment,
  job titles, supervisors, supervisors’ phone numbers)
• Two Personal References who have no past working or family relation to you
  (names, relations, addresses, phone numbers)
• If applicable, describe specialized skills, qualifications and/or training relevant to corporate wellness (e.g., experience with company health fairs, screenings, immunizations, health education, health/wellness coaching, etc.)
